When you submit a billing review request, we collect your name, organization, email address, phone number, provider type, message, and the date and time of your submission. Please do not include client health information in the form.

Review requests are stored using Lovable Cloud. Submitted requests are not publicly accessible. Information may be handled by service providers as needed to store and process your inquiry.
Retention and deletion
We retain contact information as needed to respond to and follow up on your inquiry. To request deletion of your information, call (760) 276-3312. We may need to confirm your identity before completing your request.
